I know of a few people who were told that their accounts were permanently banned, but was later given their account back as the ban was applied in error.

I also agree that using the tone of negative assurance (i.e. " no evidence to suggest the ban was applied in error" ) implies that the banning mechanism is geared towards the minimisation of Type II errors rather than Type I, which I do not agree with.
