One thing that might help, is if they made the BXP/XP from Minigames scale to virtual levels, as that would give them a good niche use (for people past 99, who have already done all the standard skilling content - so it wouldn't be making them pass through stuff more quickly).
Of course, they should only add this update to Minigames that are already (or are made so, in the same update), rewarding based on effort/participation, rather than just being in them.
I do think that it would be best if each category (PvM/Questing/Minigaming/Misc/et.c) should have two tiers of cape, similar to the way QPC and MQPC, or Comp and Trim, currently work.
Pretty much all of the suggestions you have made, would be applicable for a lower tier cape, as there really need to be a lot of proficiency-based achievements for the higher tiers of capes, rather than just unlocks, in order to bring it in-line with PvM, and really represent the depth (and challenge) of gameplay (certainly shouldn't include X# of games played - as that's already caused enough problems).
I had some suggestions for Gop Achievements, in the context of the RuneScore system (I'll redo them to fit into a 2-tier system, some point soon):
A general guideline, if going by general community feeling (going by past, as well as current attitudes), the lower tier - if it contains proficiency targets, would be placed somewhere between Tier 2 and Tier 3 of the Achievement sets (nearer to Tier 3, for most things), as those scores are roughly around the mark to show you know the basic mechanics, without requiring any of the more advanced ones, and allowing for a lot of room for error (they're all well, well below what it's possible to average on those Altars, for a decent player, so are a bit like getting a boss kill, on base mechanics).
