Who is the world guardian?

The best way to look at it is your time in the game is from your player character's perspective.

Your player character defeated Count Draynor.
Your player character slew Elvarg.
Your player character is the one that pissed off Amascut.

So your player character is the World Guardian.

All other players, from your POV, are "other adventurers". The have their own tales. They go about their business. But you are the only one to have done those things.

Part of it is also, that most of the in game quests can only be done once.
- Many quests are 1 offs because that is the only time that story or emergency needs to be done (ie: you can only kill a certain bad guy once)
- The Champion, Hero, and Legends Guilds all have an entry quest (prove yourself), but again, you are the only one that needs to that particular 'quest'. Everyone else has their own (ie: Zanik had to slay a Dragon, but she had to do a different 1 that Elvarg).

It is true that there are NPCs that are in situations that could be 'repeated':
- The Lum Cook is very incompetent. He has adventurers helping him make not just cakes, but also scrambled eggs or any of a 100 other "simple" dishes.
- Earnest (Earnest the Chicken) probably needs help every other day because of his own mishaps
- Pirate's Treasure could be any random effort to claim a hidden chest
- Lost City is doable by anyone since you still need a Dramen Staff to access Zanaris
- Recruitment Drive is a standard issue test by the Temple Knights
- Scorpion Catcher could see Thormac's scorpion's escaping regularly
- Imp Catcher could have Imps stealing Mizgog's beads every few hours
- Tears of Guthix, you just need to tell a story to Juna
- One Small Favor could see others going round about for something random
- All Fired Up so anyone can test the beacon network
- Swept Away, the witches could have a goulash addiction and use adventurers

granted, a few of the ways others could do the quest might seem a little absurd.
but in the end, some of the situations we encounter are too specific for another adventurer to get the chance to do it themselves.

granted, this is also the drawback of a long runner MMO who's had multiple chief narrators and not wanting to do 'quests' like how WoW or others do it (RS attempts to tell a real story, not a 'kill 20 bears' type quest)

Just to add one more, Icthlarin's Little Helper is canonically repeatable, since Amascut keeps on tricking/hypnotizing adventurers into stealing one of Klenter's canopic jars.

The World Guardian is very much like some of the player characters in Dragon Age. Well, at least the Grey Warden and the Inquisitor, but not Hawke. In those cases, you choose the origins for your character, but it's heavily implied that the characters whose origins you don't pick still exist in the world somehow, although they each likely die for one reason or another, some of which are outright confirmed. It's just that, by virtue of you playing characters from those origins, they go on to become the Grey Warden or the Inquisitor.

Similarly, while all these other adventurers could have potentially gone on these other quests, only you have become the World Guardian.

In case it's still not understandable.

In the perspective of the story, or by Narrative perspective, we (the players) are the world guardian. But this does not mean there are more world guardian, it just means our character that we play, by the story is the only one, at the moment. So when you do a quest, or a story related event, you are the world guardian (and the only one with the title)
