Hm, interesting thread. To point out a few things:
- It does not require access to the email to set up Authenticator, only to disable it. All you need is the login name and password.
- It doesn't smell like IP malware to me. Your account and email were fine after all. IP related malware is quite rare to start with, and wouldn't stop at 2 out of 3 accounts or focus on Runescape accounts only.
- Son 2 hasn't logged into his account recently but his bank pin was gone. This requires at least 3 days of access, suggesting whatever issue caused this problem for both of your sons started quite some time ago. I applaud your response to secure all your things as well (better safe than sorry), but I suspect the origin of this problem lies a bit further back.

Now, to get to the bottom of this:
Does son 1 have access to the account of son 2, and does son 2 have access to the account of son 1?
Is it possible that both of the accounts were used by one son, who (either intentionally or accidentally) used them for something that could have resulted in the leaking of the password and login details?

Edit:
Note that especially if son 1 accidentally got son 2 in trouble, he might not be all too eager to tell his mother, who is equally passionate about the game. It is not my intention to accuse him, it's a factor that just has to be considered here if we are to figure this out :)

I've seen those posts too, and I've been digging in quite a lot of these issues including flagging a fair few. The outcome of said flags tends to be that the security wasn't quite as extensive as the player claimed it was.

As for players claiming they got an incorrect ban: the forums and social media are filled with these guys. The reason for that is that they have nothing to lose and everything to gain. They will claim innocence because in the worst case they remain banned and in the best case they get away with cheating (which doesn't really happen but worth the try for them).

Back to the issue:
Son 2 had his bank pin disabled and Authenticator added, but he didn't move in game and his inventory was intact. Why would a hijacker disable the bank pin and take nothing, but put in the effort to put Authenticator on the account? The answer: Bank pins carry across game versions. I suspect the account of son 2 has been active in Oldschool as well.
I would be interested to hear what you find when you ask son 2 to log into his account in Oldschool. It would be a fairly clear sign that macroing occurred if you found items/stats associated with goldfarming. It would at least indicate that the macroing indeed occurred and the ban appeal will tell you whether it was caused by a hijack or not.

Hang on, I'm double-checking the bank pin information I received. It seems they don't carry across, I'm sorry for the confusion. I'm currently running a few bankpin related checks.

Nonetheless, I still suspect OSRS activity on son 2 if these two incidents are to be related. Signs to look for that indicate that the account has been used to bot on are:
- high levels in one or two gathering skills (e.g. fishing)
- large amounts of a certain item (e.g. 30.000 lobsters)
- finding the account with everything turned off (pm turned off, privacy mode turned on, etc)
- having nothing on the account other than the items needed for a specific gathering skill.
