Let your heat go down to 0%, it'll work.
It uses up one coal from your Metal Bank each time it activates.

The auto-heater really is not working for me. I've waited at 0% for more than 10 minutes, with coal in my inventory, in the ore box, and in the metal bank all at once. I have tried equipping the smithing heater, or having it in my inventory. Nothing seems to work.. I have restarted the client too. I'm at a loss.

I've replicated your scenario;
-x amount of coal in inventory
-x amount of coal in ore box
-x amount of coal in the metal bank

As expected: As my item's heat reaches 1%(?), 1 coal from my metal bank is used up to regain heat.

Just to be clear, as your character works on an item on the anvil, the yellow/orange bar (heat) depletes as you make progress.
Make sure you have coal in your metal bank, and that you have the smithing auto-heater either equipped or in your inventory.
Watch the yellow/orange bar as it reaches 1%(?), it'll automatically be filled back to about 33%. That's how the auto-heater works.

And that's exactly how the Smithing Auto-heater is supposed to work, and I know it works for other people.
It however for some unknown reason it's not working for me.
When heat reaches 1% it falls down to 0% heat, and I stay at 0% heat until the item is finished which takes about 10 minutes as I'm only getting 5 progress per hammer hit.
I've checked my coal in the metal bank I have 120 coal both before and after going to 0% heat. I've tried where I've had coal in all 3 possible sources: inventory, ore box, and Metal Bank... I have also tried having the heater in my inventory or equipped, and it doesn't change anything.

Jagex please help.


Update: I have even tried destroying the heater, and buying a new one from the shop again. Still does not work....

If the item already is at 0 % the heater won't proc, if the beta was still live I would have suggested try re-heating it for once manually.

Superheat form - I doesn't actually slow down the cooling, but it allows you to use high and med heat for longer.

That is exactly what it is used for - and combined with the superheat form you still can smith at a bit more than 2/3s of the speed an active player would have (since reheating takes time as well) - ignoring some tricks with the superheat spell of course. And especially elder rune burial armour and masterwork plates take a long, long time.
