Start by speaking to Elena, who is inside her house west of the north bank in East Ardougne. She needs you to recover her distillator which the mourners have confiscated and taken to West Ardougne. As they've also blocked up the tunnel used to gain entry during the Plague City quest, you'll need another way into the walled town.
Head over to Jerico's house, which is just north of the church in East Ardougne, and ask him about getting into West Ardougne. He'll advise that you speak to Omart about climbing over the wall - he can be found next to the Carnillean mansion south of East Ardougne Castle. Before you leave, take two pigeon cages from behind Jerico's house and search his cupboard for some birdseed.
Creating a distraction
The gate into West Ardougne is guarded by a watchtower, letting the mourners watch over that entire side of the wall. Use the birdseed on the watchtower to scatter it, then click on the pigeon cages to open them. The pigeons will fly out and distract the mourners, so go and speak to Omart who will help you climb into West Ardougne. If you need to get out again, speak to Kilron who is on the opposite side of the wall to Omart.
Infiltrating the mourner HQ
The distillator is probably in the mourners' headquarters, in the northeastern corner of West Ardougne. To get inside you'll need something to preoccupy them - grab a rotten apple from just west of the fenced area of the HQ, then squeeze through the fence and place the apple inside the stew cauldron.
Attempt to enter the HQ, and the mourners will complain they're ill and want a doctor. Go to the southwest corner of West Ardougne and you'll find Sarah the nurse's house. Enter it and search the cupboard to find a doctor's gown - put it on and return to the HQ, where you'll now be allowed inside.
Go upstairs where you'll find a sick mourner in front of a cage. Talk to the mourner about getting inside the cage for medical reasons, and kill him when he realises you're not a real doctor. Take the key dropped by the mourner, use it to unlock the cage and search the crates to find the distillator.
Testing plague samples
Return to Elena in East Ardougne and give her the distillator. After she's tested a plague sample, she'll ask you to take a fresh sample to her mentor Guidor in Varrock so he can confirm the tests. You'll also be given vials of ethenea, sulphuric broline and liquid honey for testing, but you'll need to speak to the Chemist in Rimmington to get touch paper.
Warning: Do not teleport or enter combat with the plague sample in your inventory. If you do it will break! Luckily, you can leave the plague sample in the bank and take it out again when you need it.
Getting the touch paper
With the plague sample safely banked, go to Rimmington and ask the chemist about getting touch paper, but DON'T mention the plague sample or he'll confiscate it from you. If you do it right, the chemist will give you the touch paper to give to Guidor.
Outside the chemist's workshop you'll find three people who need to be given the chemicals you're carrying. Give Da Vinci the ethenea, Hops the sulphuric broline and Chancy the liquid honey. (If you give anyone the wrong chemicals they'll lose them and you'll have to return to Elena for replacements.)
Guidor's house is in the southeastern corner of Varrock, next to the fancy dress shop. Before going to see him, purchase a set of priest robes from the clothes shop in Varrock Square. Take the plague sample out of your bank, put on the priest robes and try to open the gate next to Aubury's rune shop. The guard will search you and (provided you've done everything mentioned so far) allow you to pass. Now go into the pub here and collect the chemicals from Da Vinci, Hops and Chancy.
Go to Guidor's house - his wife will mistake you for a real priest and allow you to enter. Give Guidor the plague sample, chemicals and touch paper and he will perform his test, apparently confirming Elena's results. Head straight back to East Ardougne and give Elena the news, who will tell you to report your findings to King Lathas. Go into East Ardougne Castle and climb the stairs to find the king - after a long conversation about the plague, the quest will be complete!
